Volunteer Forms
Instructions for Out-of-State Volunteers:
1. ALL VOLUNTEERS must complete, sign and submit the following as far in advance of your scheduled time with us as possible:
- Volunteer Information Form
- Volunteer Releases
- Confidentiality Agreement
- OUT-OF-STATE NURSES: Also complete, sign and submit these forms as soon as possible:
- OUT-OF-STATE DOCTORS: Also complete these forms:
2. Send a legible copy of:
- A government issued photo ID (driver’s license or passport)
- Health insurance card
- Proof of recent (within 1 year) TB skin test
- Proof of malpractice insurance (for In-state Nurses, Physicians, counselors)
- Copy of current CPR card (nurses, doctors)
- Copy of your current Medical/nursing/social work/counseling License
- Copy of your DEA number (doctors only)
